Payment Information

Prairieland Wellness Center does not accept insurance for any services. Payment must be made in full at the time of service by cash, check or credit/debit card. Visa, Mastercard, Discover and American Express cards are accepted. A 2.7% processing fee will be added to all card transactions.

Should you wish to seek reimbursement from your insurance provider or medical spending account, you may download a copy of the Pre-Determination Form below. Please note that while some insurance providers will reimburse patients for pellets, most insurance providers, including Medicare, do not consider pellet therapy to be a covered benefit.

• Initial Consultation Fee - $150
  • This is a one-time ONLY fee charged at the initial visit to review your lab results and your medical history, and to evaluate whether you are a candidate for testosterone pellet therapy. If you are a candidate, we will develop a treatment plan for pellet therapy.

• Pellet Insertion Fees: These fees are charged at EACH pellet insertion visit
  • Bioidentical Testosterone Pellet - variable fee ($24-32 each)
    • ​Men require an average of 6-10 pellets per insertion
  • Procedure Fee - $225
