Payment Information

Prairieland Wellness Center does not submit insurance claims for any services. Payment must be made in full at the time of service by cash, check or credit card. Visa, Mastercard, Discover and American Express cards are accepted. There is a 2.7% processing fee for card transactions.

Should you wish to seek reimbursement from your insurance provider or medical spending account, you may download a copy of the Pre-Determination Form below. Please note that while some insurance providers will reimburse patients for pellets, most insurance providers, including Medicare, do not consider pellet therapy to be a covered benefit.

• Initial Consultation Fee - $150
  • This is a one-time ONLY fee charged at the initial visit to review your lab results and your medical history, and to evaluate whether you are a candidate for testosterone pellet therapy. If you are a candidate, we will develop a treatment plan for pellet therapy.
• Pellet Insertion Fees: These fees are charged at EACH pellet insertion visit
  • Bioidentical Prescription Testosterone Pellet - $15-25 each
    • ​Women require an average of 1-3 pellets per insertion
  • Procedure Fee - $225
